The holidays
are just around the corner and Hope Hospice is making plans for the annual
Memorial Christmas Tree! This year we will have two trees—one at First Federal
Savings Bank (301 E. 9th Street, Rochester) and one at the main
entrance of the Logansport Memorial Hospital (1025 Michigan Avenue). This year
Hope Hospice has ordered especially beautiful Christmas ornaments, which will
be personalized with the names of your loved ones.
We will also
be having a holiday open house at our Rochester office (1476 W. 18th
Street) on Wednesday, January 9, 2012 from 12-2 pm and one at our Logansport
Office (in the Lower Level of the Medical Building East at Logansport Memorial
Hospital) from 5-7 pm to thank our donors for their generosity and to celebrate
the honorees and families of memorialized individuals. During the open houses, the commemorative
ornaments will be given as keepsakes of this special holiday. (If you are
unable to attend, we will mail the ornament to you or the designated
recipient).
To make a
tax-deductible donation to the Hope Hospice Memorial Christmas Tree, please
complete the information below. For
donations greater than $10, one ornament per designated name will be created
unless otherwise specified.
Please note that your entire donation will help to provide
hospice care to uninsured or underinsured individuals in our community. It is
through generosity such as yours that we will be able to continue our service
at no cost to our patients and their families. In addition, as a not-for-profit,
public benefit organization, your gift is fully tax deductible. For more
